Completing and uploading a beneficiary form
The beneficiary form provides UNV with the required information to pay benefits owing to beneficiaries in the event of the death of a UN Volunteer while on assignment.
Go to explore.unv.org
Click on 'resources' tab
Download the beneficiary form
Scroll down the page to find the beneficiary form under 'Forms and documents'.
Or download the form directly from here
Log into UVP
Go to app.unv.org and log in.
Click on the task to begin
Click complete
UVP will take you to your assignment detail page.
Click complete
'Complete' button under the task 'Upload beneficiary form' will start the process.

By now, you should have a scanned copy of the completed and signed beneficiary form that you have downloaded before from 'explore.unv.org'.
Choose a file with the form on your device
Click upload
Click next
Click complete
To confirm and send the beneficiary form to UNV for review
UNV review
UNV will review the document you uploaded. If further action is needed, you will receive an email with an explanation and the task will reappear in your UVP dashboard.
