The beneficiary form provides UNV with the required information to pay benefits owing to beneficiaries in the event of the death of a UN Volunteer while on assignment.
Go to explore.unv.org
Click on 'resources' tab
Download the beneficiary form
Scroll down the page to find the beneficiary form under 'Forms and documents'.
